RISK FORMS

According to George Mason University policy, there are several required forms for all minor students registering for an MGTA course. They must be filled out, or registration will be voided. All forms are due two weeks prior to the start of the first session.

Each form needs to be filled out for each camper separately. This information includes emergency contact information, pick-up authorizations, etc. and needs to be done once per student, per semester.

These forms will be sent out once class registration is complete.
